How can you determine if a murmur is systolic or diastolic?

Determining if a murmur is systolic or diastolic involves understanding the timing of the murmur in relation to the heart's cycle. Palpating the carotid pulse is particularly useful because it allows you to synchronize the heartbeat with the timing of the murmur. When you palpate the carotid pulse, you can clearly identify whether the murmur occurs during the contraction phase (systole) or the relaxation phase (diastole) of the heart cycle.

If the murmur occurs as the pulse is felt, it is systolic; if it occurs when the pulse is not felt, then it is diastolic. This hands-on approach is especially effective in determining the nature of the murmur accurately.

Listening at the apex, using a stethoscope on the abdomen, or auscultating over the jugular vein does not provide the same direct correlation between the heart sounds and the pulse. These methods may help identify the characteristics of the murmur, such as location and pitch, but they do not allow for a clear determination of whether the murmur occurs during systole or diastole.
